TERMS AND CONDITIONS
I understand that:
Membership is required to participate in all YMCA programs unless otherwise noted.
YMCA of Long Island annual memberships expire one year from date of purchase. Any early cancellations must be due to unforeseen circumstances or medical issues. (Documentation is required; the final decision rests with the YMCA of Long Island, Inc.)
YMCA of Long Island draft memberships are continuous until the time I decide to terminate. If for any reason I wish to change the status of my membership, I must give the YMCA 30 days written notice.
The YMCA is not responsible for lost or stolen items
If during my membership, the rate is changed, the YMCA will give me 60 days advance written notice.
I agree to adhere to the following Code Of Conduct; noncompliance with this Cod may result in suspension or termination of YMCA membership privileges
Code of Conduct:
At the YMCA of Long Island, we expect staff, members and guests to behave in accordance with our mission and values at all times, respecting the rights and dignity of others. We demonstrate Respect, Responsibility, Caring and Honesty by:
Speaking in respectful tones; refraining from the use of vulgar or derogatory language; and dressing appropriately.
Resolving conflicts in a respectful, honest and caring manner; never resorting to physical contact or threatening gestures.
Respecting others by refraining from intimate behavior in public; abstaining from contact of a sexual nature.
Respecting the property of others; never engaging in theft or destruction.
Creating a safe, caring environment; never carrying illegal firearms or devices.
Participating in programs to build a healthy spirit, mind and body; never engaging in the use, sale, dispensing or possession of illegal drugs or narcotics, or the unsanctioned use of alcohol on YMCA premises.
Release of YMCA of Long Island from Liability:
I understand that:
When participating in YMCA of Long Island activities, I understand that even when every reasonable precaution is taken, accidents may still occur. I understand and expressly acknowledge that I release the YMCA of Long Island and its staff members from all liability for any injury, loss or damages, action or inaction of the YMCA of Long Island while on the premise or connected to my YMCA membership
I agree to the following as it relates to summer day camp (if applicable):
Rockwall climbing, as it pertains to summer or holiday day camp and parties at the YMCA, I acknowledge that sport climbing is an action sport and recreational activity involving travel in three dimensions and such activity is subject to mishap and even injury to participants. I understand that my child may suffer a broken limb, paralysis or fatal injury while participating in sport climbing. I will take full responsibility for and hold harmless released parties for any injury that my child may suffer or inflict upon others or their property as a result of engaging in sport climbing activities.
I give permission for my child to participate in field trips as part of summer day camp.
If my child is enrolled in Summer Day Camp, my child must adhere to the summer day camp code of conduct, which states that The Director, upon notification of the parent, may suspend or terminate all activities and participation in the program for the following misconduct.
Use of foul language or being rude and discourteous to staff and peers.
Intentionally and repeatedly leaving areas without permission.
Defacing YMCA property or field trip and off site facilities.
Refusing to follow basic safety rules.
Repeatedly engaging in fights with peers.
Intentionally injuring other children.
Bringing illegal substances including alcohol, cigarettes, or drugs.
Stealing YMCA or student property.
All camp fees must be paid in full and all necessary medical forms must be submitted for my child(ren)session prior to session start. All deposits are non-Refundable / non-transferable.
Authorization of Emergency Medical Treatment
If I or my dependent should become ill or injured during a YMCA visit, I understand that the YMCA of Long Island will contact the emergency contact designated. Should the YMCA be unable to reach that person, or I become incapacitated, the YMCA is authorized to arrange for immediate emergency treatment to ensure my health and safety.
I grant permission to the YMCA of Long Island to use photographs and videotapes taken of me for print or electronic publication for YMCA purposes.
By agreeing to these Term and Conditions, I attest that I have read and I understand all of the above and acknowledge that the same constitutes a release of liability and a waiver of my legal rights and also acknowledgement of the assumption by me of all risks arising out of my engaging in sport climbing activities.
